2025 FOOTBALL BANQUET PROGRAM

Required Athlete Paperwork

All student athletes must complete Rank One paperwork and provide a completed physical (UIL pre-participation form after 4/1/2025).

With the exception of returning varsity players, returning football athletes must complete UIL form, provide a current utility bill and copy of birth certificate to play.  If you are using your own equipment, these forms must also be completed:  1) form to use non-district issued helmet, 2) form to use non-district issued equipment (including shoulder pads).  No equipment will be issued until all paperwork is submitted.  Contact Coach Schwartz or Coach Jackson with paperwork questions.
